Day of Surgery Checklist

Please follow these guidelines for the day of your surgery:

• Please bring your insurance card(s) and picture identification.
• Be sure to have an adult present to accompany you home when your surgery is completed
• Please leave your valuables, including jewelry and watches, at home
• Biological or custodial parents are minor patients must be present in the building at all times
• Please leave jewelry and other valuables at home for safekeeping.
• A parent must sign permits for surgery if the child is under 18
• Guardians must bring written proof of guardianship on the day of surgery; without this documentation, surgery may be cancelled.  
• If your child is scheduled for surgery, we encourage you to bring his or her favorite toy for extra comfort
• Wear comfortable, loose fitting clothes
• Please do not wear contact lenses or make-up
• Please leave children not having surgery at home
• Please bring a written list of medications you are taking along with the dosages
• Please bathe or shower prior to your surgery, do not use lotions or powders
